Register a new patient

Once completed, the online registration form will generate an automatic email to the client, including detailed information about the Tool, and a link to the platform where the client can complete their registration. On registration, the client provides their electronic consent to participate. If at this point the client does not wish to participate, they will not be emailed again.

Please let the client know that they should follow the link that they have been sent.

By completing this form, you are confirming that you have gained positive opt-in consent for us to contact the owner in connection with the Antimicrobial Audit Tool project.
