Register your practice

The Responsible User of the practice should fill out the form below to create an account for the Antibiotic Audit Tool. 

Responsible User has the authority to register the veterinary practice and consent to and comply with the Antibiotic Audit Tool Terms of Use. They will often be the Quality Improvement Lead or Practice Manager. This user is responsible for the practice account and ensuring that Practice Users consent to and meet the Terms of Use

By filling out this form, you are agreeing to the following:

  • I agree that the practice can participate in the RCVS Knowledge clinical auditing tools.
  • I have the authority to sign the veterinary practice up to participate in the RCVS Knowledge clinical auditing tools.
  • I confirm I have read and understood the relevant Responsible User Information Sheet.

Registration form

Once you submit this information, it will be sent to Amplitude Clinical Outcomes at [email protected]. Amplitude will use this information to create your practice account. You should expect a response by email within two working days. RCVS Knowledge will use your email to keep you informed about the project.
