Sign Your Practice Up

To sign up for the Antimicrobial Audit Tool, you must be responsible for consenting on behalf of the practice to participate in the Antimicrobial Audit. This user is responsible on an ongoing basis to ensure that the whole practice meets the Terms of Use. Please ensure you have read both the Responsible User Information Sheet and Terms of Use.

What am I consenting for?

Please ensure you have read through and understand these consent statements:

Statements applicable for a Responsible User:
  • I agree that the practice can participate in the RCVS Knowledge clinical auditing tools.
  • I have the authority to sign the veterinary practice up to participate in the RCVS Knowledge clinical auditing tools.
  • I confirm I have read and understood the relevant Responsible User Information Sheet for the appropriate clinical auditing tool.
Statements applicable for all Practice Users:
  • I agree to participate in the RCVS Knowledge clinical auditing tools.
  • I confirm I have read and understood the relevant Practice User Information Sheet.
  • I understand that any data collected will be anonymised and remain confidential.
  • I understand these data collected may be used in presentations and publications, though this will not include any information that could identify me, my place of work, or my clients.
  • I confirm that all data submitted will be accurate to the best of my knowledge.
  • I understand that if I notice a breach of data of any kind, I will inform Amplitude and/or RCVS Knowledge immediately.
  • I understand that this platform is for clinical auditing purposes. Communication about the clinical progress of patients and potential complications, with both owners and veterinary colleagues, should continue to be managed according to the practice’s usual processes.
  • I understand that participation is voluntary and that I am free to withdraw at any time.
  • I understand that under the General Data Protection Regulations and UK Data Protection Act (2018) I can request access to the information that I provide and ask for my personal details to be removed at any time, though I understand any anonymised data will remain in the relevant clinical auditing tool that I have submitted data to.

Amplitude will use this information to create your user account. You should expect a response by email within two working days. RCVS Knowledge will use this information to keep you informed about the project.

Responsible practice user details

What is a responsible user?

A responsible user is the person who is responsible for signing up their practice to the antimicrobial audit tool. This person will be creating the practice account and so the information you input here will be linked to the practice account. The practice name and postcode will create the username for this account. This user will be able to run high level reports and create template reports for all practice users that delegate for the practice, to use.
